Howdy czhen,
Extraction/ejection related issues are usually related to the extractor...and usually to the amount of tension that it puts on the side of the case rim.
This is assuming that the gun isn't so oversprung that the slide can't make full travel. Anything above 18 pounds rating on the recoil spring is pushing it...and some choke like a pukin' buzzard with 18.
Pull the extractor and look to see if you can find the lot designation on it...It will be an alphabet character...upper case. If it's a "B" toss it after you play with the tension to see how the problem goes, and get a good extractor. Too much bend, and you'll get failures to return to battery, so go easy. A little goes a long way.
Field test after setting tension: Slide off, barrel removed...Slip the rim of a live round under the hook. Align it about where the normal firing position would be....You can see the brass trace on the breechface.
Shake the pistol up and down a few times. The round should droop slightly and stay put for several shakes. If it doesn't droop, or it it doesn't fall out of the slide after 5 or 6 vigorous movements, it's too tight. back off a hair and try again. Shoot the gun. If you get failure to return to full battery...especially on the last round...back of a bit more. You should see some improvement with both problems. If the extractor is Lot "C" or above, it's probably okay. The Bs wouldn't hold tension for more than about 500-1,000 rounds.
If this doesn't show improvement, we'll keep lookiin' 'til we figger it out. It's usually somethin' simple.

I had a Springfield Armory 1911 that was denting and tearing up the cases on ejection. Try a different magazine if you haven't already. When I bought a good magazine for the springfield it fixed the problem.

Thanks, Mr Tuner & Souris
The mag is factory with good spring tense, therefore, I've discharged this option. I thought is was a spring problem. I will check and post my results. Just in case of deciding to buy a new ejector which one or where will be a smart way.

Brownells has'em. I'll try to get back with a part number later on. Dawg-feedin' time...It takes a while with 12 of the hongry critters.
Meanwhile, run an extractor tension check. What's happening is that the
case isn't clearing the port in time to miss gettin' whacked by the slide...or the extractor is clocking. With the first, if the extractor tension is so low that it lets the case sorta roll with the punch, and doesn't kick it clear.
All sorts of weird things happen. If the slide hits it on the recoil trip, it bounces it into your face. On the return trip, and it sends it forward about 30 feet. Brass marks around the port will tell this tale.
If it's clocking, the crunch USUALLY occurs on the last round as the slide tries to feed the empty case back into the chamber because the case falls off the hook, drags across the mag follower and depresses it just enough to prevent the slide from locking. This one can get strange too. Sometimes it
can actually stuff the empty back into the magazine and still lock the slide.
Ain't quite got that one figgered out yet...but I've seen it happen.
A magazine with a rounded follower will keep the last round from dropping far enough to do that trick...most of the time...but it's a band-aid that covers the symptom but doesn't address the real problem.
