When the shell casing fails to eject and is caught in the ejection port and sticking out like a "stovepipe".

The cartridge is usually smoking, adding to the stove-pipe appearance.

I had thought it was the case when the shell was rammed by the slide into the back of the barrell (missing the bore hole) thereby jamming the slide.
