p 345?

My friend was shooting my p 345 the other day and he went to pull the trigger on the 4th shot but it didnt fire. I pulled the slide back and it didnt have a bullet in it so I checked the mag and it still had bullets.I loaded it and it finished off the mag . Can anyone tell me what causes this Ive never seen one do this?

Short of not having the magazine all the way in, I dont know.
