Anyone know of someone who makes one?? I have a full size USP and for various reasons I think it will work well for me. As for SOB injuries, I wear SOB's off to the side slightly avoiding the spinal column.
I normally wouldnt post this in general, but its such a strange request that I wanted to give it a bit of exposure.
I prefer leather, and it is for a left handed shooter USP 45 full size
Any ideas?

I am also left handed and carry a Sig P229 in a Haugen Handgun Leather (http://www.haugenhandgunleather.com) SOB holster behind the left hip (basically between the hip and the left buttock :)).
You probably don't need it to be IWB, even with just a polo shirt on the SOB offers amazing concealment. I find IWB to be far more uncomfortable than my SOB holster, YMMV.
Wearing it behind the hip instead of the middle of the back makes it amazingly comfortable to sit with and also reduces flopping that can occur when it is in the middle of the back. I've driven 400 miles with it on without a single problem.
I've also concealed a Ruger P90, and soon a Springfield XD 45 ACP in this way. I recommend a good belt and wearing an undershirt that way it keeps the cold gun off your body and your body oil off the gun :D.
Haugen makes excellent holsters, they take 4-8 weeks but are well worth it. The holster is molded to your model of firearm so it fits the weapon extremely well, holds it against the body well and minimizes printing.
