Looks like your basic 1-inch body flashlight in a clamp, offset to the left of the magazine tube on a Mossberg 500. A cobbled-together rig from available off-the-shelf parts in other words.

No reason it can't work, but incandescent bulbs even in a shock-mounted flashlight head (a la' SureFire) tend to have spotty lifetimes when mounted on a 12 gauge. Were I to do such a thing I'd use an LED light if i could. Even so the batteries, tailcaps, switches etc. get pounded pretty thoroughly by recoil if you don't remove the light for normal practice.
