Basically the constrict the muzzle, so a tighter choke will put more pellets on target at a given range, where a looser choke will allow them to spread more at the same range.
Really tight chokes are turkey and full, the modified is kind of the middle, with cylinder at the loose end. There are others that I haven't mentioned, but I hope that gives you the gist of it.

Different chokes provide different patterns of your shot.... the more 'open' the choke, the wider the pattern at the same distance....

Choke is the constriction at the muzzle end of the barrel. It can range from Cylinder (actually no choke at all), to extra Full (like a turkey choke. The purpose of tighter chokes is to try and keep pellets together more, as mentioned above, for shots ranging further away. Twelve gauge chokes can typically be found in .005 increments ranging from .000 to .045. However, not all bores are exactly the same size, so you need to pattern your gun to determine if the designation on the choke is accurate or not.
Close games like Skeet, or NE grouse hunting usually use more open chokes due to the closeness of their respective targets. Trap, 5-stand and sporting clays typically use chokes of a medium constriction for targets out to 40-45 yards. Handicap trap, FITASC, Bunker, etc will usually rely on tighter chokes as their targets can be even further away.
