What era are you looking for? Different places carry different eras.
Dixie Gun Works pretty much can help with any era pre-1900, back to the revolution.

i remember seeing in this forum somewhere a picture of an old cartridge box... it was a piece of wood with seven holes drilled in it... six were for the ammo, the seventh was for the caps. don't know if you were looking to make a box or buy it, but FWIW...

Dixie is OK, but I would go to C&D Jarnagin at http://www.jarnaginco.com/
one of my friends make his own paper boxe with a photoshop project.
you can find the pictures of it here: http://poudrenoire.forumactif.com/rvolver-f6/mes-cartouches-papier-en-boite-t12977.htm
and the photoshop file here http://www.did-art.fr/boite_cartouches_did_2.rar
(i'm pretty sure that you can use it with "the gimp")http://i52.servimg.com/u/f52/12/52/59/26/colt110.jpg
I made a tutorial to explain my way to make paper cartrides, i hope you'll find it interesting. it's here: http://urrp.free.fr/poudre_noire/cartouches_papier/
