I don't yet consider this "good" shooting, just adequate.

Going to take a lot more practice grouping double action, semi-rapid fire.

Target's about 5", 12 shots (and, yes, the wadcutters sometimes keyhole)

I don't see a keyhole. A little tipping or yawing, yes. Gil Hebard said the Model 52 would show that on about 20% of shots, but they were in the group.

If that's a three shot hole, I'd say it was because of an unsupported target (no backer) and the paper just ripped. Look at the shots center low on the left of the line. That's just 2 shots and looks similar.

On numerous other targets I get a holes that are the perfect image of a sideways wadcutter, but maybe it's not keyholing then either.

It doesn't matter to me since I just target shoot with them and don't carry them.
