Which Form to use? 1st time Applicant

So I have been scouring this forum and the all mighty Google and am still unclear on which Form to fill out a Form 1 or 4 and a 5320.20??
I want to acquire a 10.5" LMT AR15 upper and have a few questions.
I will be ordering off the internet from what I understand I need to fill out a Form 1 with a 5320.20 with fingerprint cards 2x and a check for $200? Is that correct?
Do I need to wait for all the paperwork to be approved before I can order it?
Do I need to ship the upper to an FFL or can it still be sent to my home?
If I can't order the upper first what do I use to list the measurements, just 10.5" barrel, .223 cal, and LMT as manufacturer?

It sounds like you're going to make a Short Barrel Rifle.
Here's the low down and dirty:
You need to fill out and get a Form 1 approved first.* You Need to list your Barrel length as 10.5" and your overall length is the buttstock to the muzzle. How will you measure this? Technically, you can't. Because if you slap that upper on your lower to send that measurement, you made an un-registered SBR.
You need to measure your overall length of your rifle, buttstock to muzzle (with stock fully extended) with a KNOWN barrel length. Then subtract the barrel length difference between your LEGAL rifle, and the SBR you are going to build. That is the overall length in your Form 1.
Presuming you are building this as an individual you need to mail off your:
Form 1 (In duplicate, each Printed front and back on one sheet,each with original ink signatures)
5330.20 Certification of compliance
Fingerprint cards (two of them)
Passport size photo attached to each form 1
Chief Law Enforcement sign off on each form 1
Check for $200.
The upper can be delivered directly to your home. However, if you don't have a LEGAL lower that can mate with that upper, and you have a firearm that the upper FITS TO, you have "Constructive Posession of an Illegal SBR."
A lot of people build an AR PISTOL, then when the form 1 gets approved, build it into a SBR.
The manufacturer is YOU. The SBR needs to be engraved with your name, City, and State. LMT made the upper, BRAND B made the lower, and YOU put them together to make the short barrel rifle.

*Form 4's are for transfers of Title 2 firearms that are ALREADY BUILT.

Thanks RJ! That is a huge help.
Do I include the flash hider into the measurement? So on a 20" AR15 i would subtract 9.5" to get my hypothetical length?
Where can I print off the forms, I went to the ATF website but it would not let me download them or have them mailed to me.

You're welcome.
Flash hider IS NOT part of the measurement if it isn't pinned and welded. No reason to pin and weld a flash hider on a SBR.
