Photos on a form 4 tax paid transfer

I a buying a suppressor and need to know how to attach the photo glue or staple? Can anyone put up a link for full explanation of the forms so others can buy suppressors. I had a mistake on my first go around, they did not disclose what, and waited 6 months only to have my gun shop call and be told finally that their was a mistake keeping me from approval and to start over. Major PITA!!! So in order to keep other from committing the same error lets get the info out there.

A mistake on a Form 4 is handled very quickly by the ATF.
What usually happens (usually) is that a clerical error on the part of the individual filling out the form 4 is identified by the examiner, the forms are returned to the person/gunstore selling the item and the error is corrected and sent back to ATF examiner directly. The corrected form is processed and the stamped form is sent out usually (again usually) in very short order.
Oh and the error is always identified by the examiner so it can be corrected.
If your gunshop is telling you that you have to start the whole process over (to include payment of the 200 dollar tax), I'd find another gunshop to do business with as they (the gunshop) are running a game on you.
In fact I find it very hard to believe that your gunshop is allowing you to fill out the forms, especially if you have no experience in doing so (not that its hard, it usually only takes about 3 minutes to fill one out).

They show completed form 4 (albeit for a trust) and that should help you figure out what has to filled out.
With respect to attaching photo's, I use a glue stick. Never had a problem with them falling off.

I used double sided tape on mine, if they are telling you to start over I would also suggest a new shop unfortunately some places only have a hand full of title II (class 3/SOT) dealers and they can be jerks or completely ignorant about the products they sell and get away with it because they are the only game in town or for 100 miles.

If someone is coming to me convinced that they know the process better than I do and they want to do their own forms, I am not going to argue with them.
(Nor will I fix the forms if there is an issue.)

I put a loop of tape on the backside of the photo then wide clear packing tape over it.
