How are integral suppressors manufactured?

I know it's going to be different for each kind of build but using a .223 barrel as an example, would it be basically taking a larger diameter than necessary 20" barrel blank, cutting a 12" .223 barrel in it on one side with the usual headspacing and bolt matching cuts (not sure of exact terms) then cutting out a much larger cavity for the rest of the length, inserting a monocore buffer or stackable buffers, then attaching the endcap?
In this example, how would you go about cutting it for the gas expansion in to the gas block and how would one go about tuning this build? An adjustable gas block or start with smaller holes and drill out as testing goes along?

A relatively thin barrel is made, then holes are drilled in carefully between the lands and the entire thing is sleeved in a large diameter tube with baffles built in down the entire length, sealed at the front and back with endcaps.

That makes much more sense than my premise and cleaning it would be a lot easier to do than my idea (prying out a caked on monocore would be hell). Thanks.

Or you make a short barrel, then build a suppressor on the end and perm attach it. There are lots of examples out there. Some of our members make their own.

If there are no baffles in front of the muzzle, then I think this design would work poorly.

True, I forgot about that part. The sleeve/baffle assembly would have to extend beyond the inner barrel.
