I would say that a round fired out of battery or there was a squib load that made it just far enough for a second round to chamber and then a round was fired behind it.

I would bet on the former before the latter.

Believe it or not, lots of people don't do Facebook. I've never even seen a Facebook page and doubt I ever will.

Well what happened is, a case let go due to high pressure or a weak case.

The M16/AR-15 has no planned gas vent plan, unlike all other rifles of the past.

If you blow a case, it does exactly that to the bolt carrier, upper, and magazine.
Because that is the only way for the high pressure gas to get outside the action when a case lets go.

Perfectly normal gas escape route.
When there isn't one, it makes one.
