I have used Galco combat master and safariland 518 holsters in the past with good sucsess but now I need a holster for a 4" S&W 28. Galco does not make a combat master for revolvers and the one problem I had with the 518 is that it seems to push the gun away from the body but it was very comfortable so I wouldn't mind a paddle holster with an FBI cant that still holds the gun close to the body or if its good I would think about another style of holster.

If it isn't for concealed wear, the M28 seems a natural for the basic Tom Threepersons holster, named for the part-Cherokee lawman of the 1920's. Made originally by Myers, it is now best done by El Paso Saddlery.

The old Bianchi M5 and the vintage Safariland M29 Thumbsnap are also superior choices, if one can find the latter used at a gunshow. These are basically the Threepersons with a thumbsnap feature. El Paso also offers this style.

I think Kramer Leather may have some good options for your Model 28, too.

Galco has the Phoenix which is strong side and/or
cross draw. Got one recently from Arizona Gunrunner. I have 3 such.

For CCW I like the Texas Heritage from Tucker Gunleather. It's a tuckable IWB with adjustable rake and with supports far enough apart to conceal well and give great control. It holds the grip in towards your body yet is easy to use.

I've had very good success with both Mitch Rosen and Kramer Leather: top quality and customer service.
