What would cause the top lever on a barely used side by side shotgun to be substantially to the RIGHT of centre?

Over time the parts wear-in or mate together. The top lever will move to the center just as denster has said.
On a used gun if the lever is far over to the left, you know some action is needed.

It's when the lever is left of center that you need to be concerned as the gun is showing signs of wear.
Yes, that’s what I’m aware of. Most the time you see them a bit to the right of centre, but this one was considerably more than I had ever seen before.

And isn't just SxS shotguns either. This goes for any break action shotgun even if it is a single shot or O/U.

I dare you to find the top break lever on my Ljutic. Or an, Alfermann, Seitz, Bowen, or DeVault. :D
These all have locking systems that are actuated by either pulling a button in front of the trigger guard back (The Ljutic; it is actually the front of the locking block.) or pushing the whole trigger guard back, or the back of the trigger guard up.

I like watching people try to figure out how to open my Ljutic when they look at it.

Oh, and the H& R Toppers have a latch button next to the hammer you push down.

Then there's the older (usually European) break opens that have an underlever or side lever.

@entropy you own some oddballs then :p Most of my break action shotguns have a center lever on top. Now my old Savage 20 gauge as the lever on the side of the receiver.

All kidding aside not all break actions have a lever on top of the receiver. But for those that do, it is common for them to point to the right.

I've seen neat old stuff like that. I've seen a side by side where you open it by pushing one of the triggers backwards. Seen one where a metal piece on the bottom of the fore arm swings to the side

The Ljutic is not old, but rather one of the top trap guns made.
Joel Etchen Guns Just sold one for $8500!!
