Because of a lack of a physical behind, I have had trouble keeping my pants up (don’t laugh) let alone with a full size defensive pistol. I’m 5’6” and weigh in at 157 and no part of that 157 is in the Butt.
With a good belt and Perry suspenders I can make it through the day with a J frame in my pocket or a Sig P365 worn OWB. Any of you folks have the same problem and how do you cope with it.
 
I also suffer from nunatall and to add intrigue mine comes with a mild metals allergy! I swapped to suspenders only. IWB holsters don’t work out well for me but pocket holsters do OK.

I once made a compression undershirt holster for when riding motorcycles. It put the pistol between my shoulder blades at the base of my neck. I liked it but if I had ever gone down it probably would have killed me. I’m considering doing the same thing again but using leather and wearing it between shirts. With my new line of work I need deep conceal but I also need to be able to put heavy boxes right against my body to carry them.

Ahh the old Hank Hill problem. Every male in my family has that and I'm getting there too. Suspenders or a shoulder holster is the cure.
 
I suffer from these symptoms also.
I don't have a good cure.

I wear a real gunbelt. Mine is Azula. I have two of them, black and brown.
. I have also chosen lightweight pistols. The lcp causes hardly any extra sag. The new Hellcat drags a little more, but tolerable. Today, I am going to carry the hellcat 13+1 and no extra mag.

Called DGS or diminished glute syndrome. Had it for years. Suspenders, particularly good are those from Duluth Trading.
 
I'm pretty typically sized- 6'1" 185#, I've been carrying since I was scrawny 155# with no backside to speak of. My solution has been to put some nonslip material on the surface that touches my body. I use moleskin that I buy a roll of from Amazon. It usually lasts 6 months and I just replace it.

Not perfect but it helps. Also, a belt that doesn't rely on getting the right fit with a hole through it. You want a belt with adjustability so you can pull it snug regardless of exactly what you're wearing. Never found a belt with set sizing holes that works for me. I wear a very rigid 5.11 belt , I can pull it tighter if it's slipping down . works for me, I carry a g30 that isn't slim or light , my loaded magazine weights as much as some small guns.
 
My wife says I am getting Clint Eastwood butt as I get older. One thing to consider is a belly and type. Held up by elastic and pull pants over it so it’s independent of your pants and belt and is IWB. Put the gun where you want to carry. It works, hot in the summer but it works. They are $20 and worth a try.

Held up by elastic and pull pants over it so it’s independent of your pants and belt and is IWB. Put the gun where you want to carry. It works, hot in the summer but it works. They are $20 and worth a try.

5'8" here and roll at 135lbs

Appendix carry for concealment works for me. Strong side just requires a good belt and holster loops that are spread out a bit.
 
I think it's more my hips that hold up my belt/pants, not what little donkey I've got. I do have fitting pants (I only wear jeans) that can fall if I don't have a belt on, but I've never had an issue with a belt, and never go without one. The two belts I use for carry are both pretty stiff and heavy leather; one of them is a basketweave law-enforcement "under-belt", the one worn through the belt loops, under the officer's "gear" belt.
 
I had a boss who literally could put on bicycle tights and the rear would sag. He used a 1/4” thick, 1.25” wide gun belt from Bullhide belts and a set of Carhartt suspenders. (He told me about Bullhide, and I’ve been using one of their belts for years.)
