This is why having a written set of reloading procedures and an associated check-list that is completed for each session is so crucial.
A squib is not a catastrophe, but it is the entre to a sequence of events that can lead to permanent disability and we should never assume it will be detected by the "immediate action" that typically follows a misfire.
