Who could put choke tubes in my single shot?

I have an H&R 12 ga. that my wife bought me years ago, and it is great fun to squirrel hunt or duck hunt with, but the full choke limits what I can do with it. For years I have wanted to put choke tubes, or even a Polychoke in it to make it a little more versatile.

Who could do this that is reliable (I do not want them off center) and about what would it run?
 
It may or may not be worth the expense, but Trulock could do it. (You will have to decide for your self about the sentimal value of the gun vs. just spending the money on a second gun.) From what I understand Trulock is the top of the line for removable chokes. I have been on the fence for a couple years about having an older 870 Wingmaster with a 30" barrel full choke barrel fitted with their choke system.
I just looked at your specific firearm, and it is a lot less than mine. I say go for it and report back on your patterns.
