Questions for anyone who's bought a recent production, late 2009 or 2010, 9mm DA/SA CZ75B:

Are they shipping with standard flat panel grips or with the
military-style thumb rest grips?

Does CZ still proof the barrels, slides and frames?

Are the barrel, slide and frame all serial numbered?

Is $495 OTD for a NIB 9mm CZ75B with two sixteen round mags a good price?

Flat grips, yes.

Barrel, slide and frame are indeed numbered as shown.

Mine came with a factory test target.

I found a NIB 2010 production 9mm DA/SA CZ75B at a local shop. I don't know how representative a single example can be, but it had flat grips, serial number on frame, slide and barrel but no proof marks. It also had a trigger like the ones used on the stainless models.
 
+1 on the Hogue Grip. I took off the military style grips that came with my imported CZ and replaced them with the Houge grip. Vast improvement over those bulky plastic grips.
 
I like the Hogue grip too. Get the grip screws to go with it also...they are worth the extra couple of bucks. The VZs are sort of pricey but I think their 1911 grips are pretty good so the CZ ones should be the same. Never tried them on the CZ though.
