I've been using 15-round MecGar hi-caps with my P228 since I got it 3 or 4 years ago. Never had a single issue with them.
They're very much worth it, even if you have to pay $70 apiece for them like I did. :)
J is the old, 19th century .318 round.
JS is the more modern variant, which you want.
JRS is the rimmed version of JS, and I haven't personally seen any guns that fire it. I guess they exist, though...

Actually FALs have charging handles on the right side of the receiver. The inch-pattern ones fold down like on the CETME, while the metric-pattern ones are just little knobs.
That's about right. If you look around, you might be able to find one for around $1,800.
They're big and they're (somewhat) rare. Also, they're the only handguns designed for offense from the start. :D

The D stamp doesn't say anything about the bore diameter. It merely means that the chamber can accept the D-166 ammunition.
There are four (or five) commercial brands of 7.62x54R that I know of. Wolf makes a 148gr. and a 200gr. round, there's the Norma stuff that sells for $35/20ct...
