4" Revolver CCW: IWB or OWB?

No, it doesn't bother the arm.

It's basicaly behind the arm, not under it, so your elbow would only hit it if you were trying to touch your elbows behind your back.
To answer your question: The 4" K-frame is a full-sized service gun, but not what I would call large. With the right grips (which are EVERYTHING when carrying a revolver) it feels pretty sleek. Many would tell you that anything larger than an airweight J-frame is too big to carry. I totally disagree.
I am NOT a big guy, and I carry a pretty darn big revolver no problem. If, however, I'm going to be in shorts and a T-shirt, I have to switch to a 3" K-frame (the four incher would be fine, I just don't have one) with a round butt and boot grips. Easier to carry that than a full sized 1911, although that's also a very good choice.
If it's a blued gun, and you are a sweaty bugger (like me) than you should be careful carrying it all day in the summer. Otherwise, there's not much keeping you from carrying a big gun. What matters much, MUCH more is if you can SHOOT that gun well.

Thanks for the replies; very helpful. I have a Dan Wesson .357 with a 6" barrel, which is at least L-frame size that I use for hunting and target shooting. I have a Uncle Mike's waist holster that I use to wear it in the field.

So, today, in order to mimic the effect of a high rise holster, I strapped on the Uncle Mike holster to about where my belly button is and placed the gun with a slightly forward cant at 2:30 to 2:45. VERY comfortable place to carry the gun. Unlike wearing it much lower at 3:00 o'clock, in this position the grip does not constantly hit my arm. I can keep my arms to my side without the gun bumping into my strongside arm. Also, it's very concealable. I put on an oversized camo shirt and there is only a minor bulge where the gun is at; can't even tell it's a gun so that's no problem at all. The shirt covers the entire gun all the way to the end of the muzzle and when I lean forward, the gun is kept tight to my body while the shirt fans out, so there's no print.

Definately not a CCW though since the very end of the 6" barrel bumps into my thigh when I lift my leg up. Also, while loaded the gun weighs 40 something ounces.

So with a 4" K-frame or Security Six sized revolver I really should have no problem carrying it high rise OWB.

What would be the advantage of carrying it IWB? You still have to cover it up with a shirt. The way I see it you can at least carry it while driving OWB, but not IWB.
