870 Question

I have had a Remington 870 Express in 20ga for about two years now, although I only recently began to shoot clays with it. Anyways my question relates to extraction of the shell, it has happened to me three times know and as this gun also double duties as my HD gun I am pretty worried about it. What happens is the shell just plain fails to extract, the extractor is either jumping the rim of the shell or tearing through it. What I am wondering is what could be causing this? Also what can I do to fix this?
 
Thanks all, I don't believe it is a cleaning problem as I put more effort into cleaning that gun than any other gun I own. That thing is immaculate. I will try to polish the chamber with the steel wool etc. Thanks again.

Here's what I'd do, after polishing out the chamber and still having the prob.....

If this happens with only one brand/lot of ammo, I'd use something else.

Then, I'd dismount the extractor and make sure that grunge hadn't impeded the movement of the "Hook" so it couldn't get as good a bite.

I'd carefully examine the hook for the tip breaking off or other damage.

If that didn't clear it up, I'd find a good smith.

How would I go about taking the bolt all out and apart to dismount the extractor? If it comes to that I don't want to screw it up, and I don't remember weather or not that was in the owners manual or not.
 
After making SURE the shotgun's unloaded, remove the barrel by removing the mag cap and pulling forward. Next, press on the shall latches inside the receiver and pull the forearm assembly forward until the bolt clears the receiver.

Next, press on the front of the extractor and rotate it away from the bolt body. Go slow, there's a spring and plunger inside that are small and tough to find if they get away from you. Clean, lube lightly, and reassemble.

Last time it happened I was, I pulled all of it out of my vest and switched to the Remington Express Long Range in my other pocket.
