Crushed 1911 case mouths

I think I've seen posts that go around this one, but I can't say I've seen the one that is just what's going on with my Officer-sized Para Companion LDA.
The empties leave the gun, apparently properly, but a fair number of time, the last one before slide lock ends up with the case mouth crushed in in two places, 170 degrees apart. It's so striking that I've actually stood there and taken note of the bent brass being specific to the last round.
If it happens with different kinds of ammunition, I haven't noticed.
This is one with a later (2005) PXT slide.
The gun runs fine and I trust it, but this just can't be right.
What the heck?

It's the extractor. Either not enough tension or it's clocking and dropping the case. Almost always happens on the last round. If it's clocking, and also has good tension, sometimes it'll actually stuff the empty case back in the magazine.
 
Oh my goodness!!!! Tuner, where was the internet and your help when I built my first couple 1911 types? I actually built one that would put the fired empty back in the magazine every couple magazines on or about try number 3 at getting a 'custom 1911 for 1/4 the price' LOL. SO many people laughed at me, said it wasn't possible, what was I smoking and so on. I fixed that gun, and got it to work every time on time, but I wasn't exactly sure how.

Followup

It's kind of hard to do much with a Para PXT extractor, which has a concentric plunger to drive a pivoted claw end. I took mine apart, cleaned it, and put it back together.
I did not see any signs of clocking.
I took it down to the range and put a quick hundred through it. I only found one case to be dinged, and it was hit less hard than previously.
