I had that happen when my homemade lube was too sticky.
Wads were stuck to the back of the ball - significantly throwing off the accuracy.
 
I recall years ago shooting a percussion revolver at the range using wonder wads under the bullets. Upon retrieving my target, one of the wads was stuck perfectly in the bullet hole in the target.

For sure, never seen anything like that before. Light charge, bullet seated way deep, and lots of lube I guess? I tried lubing over the ball one time in my life, after I got my first C&B revolver. I says to myself I says: "that's enough of that". This is sure an extreme example of way way way way way way way too much lube.

I've never had dry wads stick to a ball, but for sure waxed wads. If the gun stays loaded long enough, like months, the wonder wads will stick. The lube on them seems to interact with lead. If you put wonder wads and balls together in a container, the balls get heavily corroded or oxidized pretty soon.
 
Mike 56 recommended using Gojo original formula hand cleaner over the ball as a patch lube.
It's also good for cleaning BP guns.
He also liked Goop, both are white waterless hand cleaners without pumice.

First I've seen of this. Might want to use something not quite as thick. I've used 70/30 veggie oil/wax cooked together for near ever and never had this happen. Use a cooking syringe and just apply it around the ball. No need to fill the cylinder.
 
I used to produce targets that look like that, using beewax/crisco over the ball. It told me that I did not need to completely cover the ball with lube. I now use about 1/20th as much over the ball as I used to, just wiping the flake of lube over the face of the cylinder to get a bit of lube on the face itself. A little bit of lube naturally scrapes off and clings to the mouth of the chambers as I do this. The lube flakes last a lot longer and I can still go 6 stages at a cowboy action match (30 shots per revolver) without cleaning or fouling or accuracy problems.
 
Yes I was using a ton of lube over the ball. I’ve been working with different mixes to see what I can get to stay in the cylinders as it’s being fired. I’ve had lots of trouble in the summer keeping the lube to stay. I will go as high as 70% wax to keep it from blowing out on a hot summers day. This stuff was 50% wax, 40% Crisco , 10% vegetable oil. Temp at the range was just above 50. Seemed to stuck pretty good. I also use wonder wads and many times just use wonder wads only. I like to play with different combos and see what it does for keeping the barrel clean and accuracy.
 
Bees wax & olive oil mix according to temperature. Cold more olive oil , Hot more bees wax.
