They did not use any trademarks or insignias. The court ruled that the name itself was trademarked and an insignia.
No, but they did issue press releases and formal documents as though they were the ICHV organization itself. The simple act of registering the name isn't what got them in hot water (and even that seems pretty ticky-tack--ISRA probably needed better lawyers).
