Not locking back after last round?

Many reasons.
Here are a few common ones.

Damaged or worn slide stop.
Damaged or worn slide stop notch in the slide.
Weak mag spring not able to push the slide stop up.
Bent, bent, or worn magazine follower not engaging the slide stop.
Weak ammo not getting the slide all the way open.

You left off the most obvious (and common) cause for right-handed shooters: the operator's thumb riding the slide release.

Here's a quick way to diagnose the problem: shoot the gun in your left hand. Does the problem go away and you can't recreate it? Then it's your thumb. If it persists, then explore the magazines.

That happened to me when I switched to the thumbs forward grip. I't took me a few minutes to figure it out because i also had a bunch of FTF with remmy bulk ammo. The slide not locking back AND a bunch of duds really confused me.
