I have a couple of thoughts.
1) The firing pin bushing might be out of the frame slightly
2).The star extractor might be slightly bent and the gap is too small for the round to rotate
3) The crane might be slightly bent and the gap is too small for the round to rotate
4) As everyone mentioned the extractor rod may be coming loose
