ammo coming out of magazine in pouch

i have a beretta nano, i am using a knife sheath as a mag pouch. this morning as i was putting on my gun and sheath i noticed a rattle. i opened my sheath and a round had came out of the mag and was at the bottom of the sheath.:( the mag fits good in the sheath and isn't loose inside so i don't know what's going on. now i have carried this sheath with the mag for about 3 months, should i just check the pouch each day or is it cause i'm not using a real mag pouch? thanks for any help
 
I don't have a Nano, but I've carried Kel-Tec mags (9mm and .32) in knife sheathes quite a lot, and have never had this happen. It's hard to say without visually inspecting your setup, but I can't imagine a round, under pressure, sliding out unless there was both a "forward-and-back" friction action going on in there, and room enough for it.

If there is a little room in there, maybe you could affix a plastic "spacer", perhaps fashioned from a piece of thick wire tie ("zip strip") along the leading inside edge of the sheath.

EDIT: I just looked at BSA1's post above, and what he says certainly sounds plausible.
 
Same thing happens to my Sig P938 spare mag in the mag pocket of my 5.11 trousers.
Havn't found a solution, other than using a real mag pouch specifically made for the mag.
 
Some "single-stack" magazines don't seem to hold the top round very tightly. I've had Kahr pistols where it will inch its way out on its own. In conjunction with a loose-fitting, flexible pouch, I'm not surprised you have problems. The best solution is a snug-fitting, more rigid pouch.
