Reminds me of the all too common S&W 916A receiver failure. There's no excuse for this type of failure, IMHO.

For myself, no. I've been shooting 1911's for nearly 30 yrs and I have never had an issue with the grip safety, nor have I known anyone who did. I would not disable the grip safety in any way.
