I don't know if you can tell what's going on here, but there is a kydex tab where the trigger guard is, and if gun is holstered crookedly (easy to do at 5oclock) the tab can get lodged into the trigger guard. VERY UNSAFE. (this is for the kahr pm9)

I don't know if you can see what's going on here, but if gun goes in crooked, the kydex flap by the trigger could get lodged in the trigger guard.. theses minotaur holster frames should be recalled. (this one is for a kahr pm9)
