There is, quite possibly, a problem with your mag. followers. You just might not know it yet!
After having 2 out of 3 followers broken on my CW-40 mags, I called Kahr to inquire about replacements.
As soon as the guy heard me say that I had broken followers, he knew that I was talking about the .40 mags. It seems that their supplier changed the resin recipie which has lead to problems. I've been told that it's now been corrected.
However, you should examine your .40 followers for cracking. Contacting Kahr about it should result in new followers being sent your way.
Three new followers should arrive at my place later this week.
