Last week when I was loading, I would get one of these ocassionally, but today, I am getting them very often, maybe 1 in 5. What am I doing wrong or missing?

Raise the wad guide up a little.
It's allowing the shell lifter to raise the hull up out of the clamp collet and resizing the rim.
Then the de-priming punch is bulging the base because the rim is not in the groove in the collet clamp.

Thank you for your reply. Do you mean the plastic thing I set the wad in before it gets pushed into the shell?

The wad guide should be locked in the groove on the arm it sits on.
