It is not a case issue. The case will drop in properly every time, without the bullet. Once the bullet is seated, it will not drop properly. The mock cartridge still works great in the real barrel. The bullet is .400.

I have shot numerous rounds of .40 SW from Bulk Ammo (bulkammo.com). I have noted no problems. I took a look at their 9mm, and found scathing reviews on their 9mm. From the reviews, it sounds like a real production problem.

The P209XS is the expander w/o the tip. The P209XT1 is the expander with the polymer tip. This info is not readily available on a lot of ammo web sites.
