I found in my experience this has been caused by a flake of powder which has fallen into the cup containing the pin (and primer). The half moon is the imprint of the compressed flake of powder on the primer. Just clean the head of the pin. Next time it happens, look there first.
I also use the "newer" RCBS hand primer for my bottleneck cases except 223. I use the Dillon for them. I used to use the on-press primer tube setup with my ancient Pacific 07 press. After 35 years with that press/primer setup often going out of whack, I'm glad that I got the hand primer...

I found that putting the powder baffle from my RCBS Uniflow into my Pro Auto Disk helps to keep the powder charges consistent. I use the double disk for Varget in 223 and the charges rarely vary more than .1 grain. Flake powders like Unique also measure much better with the baffle in place.
