.410 Mosseberg problem

My wife has a .410 Mossberg Cruiser pump shotgun. We were loading it to go shooting the other day and noticed that when the action is cycled, two shells come out of the tube into the chamber and jam the action. I've noticed that when the tube is full, I can push on the leading shell and dislodge it out of the tube. Anyone know how to fix this? Thanks.

Could it be a bent shell catch on the receiver end of the mag tube?
