Best one I ever saw was a guy shooting a .338 win mag round through his 7mm rem mag. KABOOM!

I accidentally chambered a .50BMG round into my 10/22 one time. It reamed out the barrel so now I have a rotory magazine .50BMG autoloader.

Supposedly, possible caliber confusion is also the reason the AK74 has the shallow grove on each side of the stock: So the barely-literate conscripts could tell with either a glance or by touch if they had a -47 or -74 in their hands.
