I had to do some serious digging because the Office of the Inspector General's report conveniently redacts (i.e. covers up) this disgusting woman's name in all SIXTY THREE pages.
TIME LINE:
November 24, 2008: The OIG report is made and approved (it apparently only recently became public now that she is long gone

January 23, 2009- ALMOST TWO MONTHS TO THE DAY after the OIG report is approved:
