I think federal law pretty much guarantees medical record privacy.

Federal laws do address privacy of medical records but the number and size of the exceptions allowed are considerable. It would be simple for new legislation or even beaurocratic fiefdom regulation to allow the use of mental illness history by the BATF FBI etc. for firearm purchases. The only medical history that is virtually guranteed to remain confidential is HIV status. HIV is the only pathogen in history to have a small army of lawyers working in its favor.
