I purchased a suppressor last year, and it was successfully transferred to my trust in October. Yesterday I got a letter from the class 3 I used for the transfer, and he is stating I need to fill out a 4473 to complete my transfer :what: I have the forms and the ATF stamp, etc so I can see no reason why I need to fill this form out several months after the fact.

A couple of questions:
1) I thought the whole point of a trust is to avoid 4473/cleo signoff. Why would a successful trust transfer need a 4473?
2) why would he wait 6 months before asking for this?
3) what should I do?

I now live over 300 miles from the cl3 and frankly don't want to deal with him anymore (it was a long and difficult process, most of which stemmed from the cl3 dealer). You're help/advice is appreciated.

The 4473 should have been filled out when you purchased the suppressor. The dealer made a mistake.

I'm curious how they discovered they made a mistake after 6 months.

You avoid the prints and the CLEO sign off, but the 4473 still needs to be filled out for everything that leaves the shop. Section 22 of the 4473 is a check box that basically states "No background check was necessary because only NFA items were transferred." That long wait was the gov't doing a background check, so a NICS check is not needed, but you should have still filled out the 4473 when you picked up the can. Dealer's mistake.

2) why would he wait 6 months before asking for this?

He either just caught the mistake himself or someone else caught it for him and he is trying to get things back in order.

I understand your reluctance to drive back to the dealer. I'd call and have him send you a blank 4473 that you can fill out and mail back to him with a copy of your Driver License.

correct

In order to avoid getting into trouble with the ATF, most dealers check inventories against paperwork a couple times a year. I'm guessing he either caught his mistake or perhaps it was caught at an ATF inspection.
