Pie Chart/Target - POI Corrections

I know I have seen this recently, thought I saved it but can't find it. Worked the search button over pretty good with no luck.

There was a thread recently that had a link to an older thread that contained a chart. Kind of like a dart board looking target. You find the spot on the target where your POI is and it would list a one or more possibles reasons/corrections.

Anyone have a copy or link to this?
