I forget if I shared this info before. Sorry if I did.
But I got an email in answer to my question from the FCSA (Fifty Caliber Shooters ass'n) at wwwDOTfcsaDOTorg, which asked if you need to shoot a 50 with your mouth open.

Here is the info:

cpileri: "I searched and found only conjecture and unreferenced statements that you must (or should) fire the 50bmg with your mouth open to equalize pressure in your inner ears.
First, is it true?
Next, is it (i.e. mouth open) necessary when wearing earplugs or earmuffs? When wearing a combination of both?"

FCSA response:
"Ummm....no. Otherwise I'd have been deaf since 1994.

Double ear pro is recommended though. AND, to make sure that's crystal clear to the students at last week's (ed.removed) Class on .50 caliber, the (removed) Instructor pointed out that "double ear pro" doesn't mean TWO sets of foam pluggies in the ears. You'd have to drive them into your head with an ice pick. Best use a set of foamies AND a set of ear muffs.

Now, if they get tired of whacking at the target with a .50 to no effect and call in a JDAMs strike, then opening your mouth & plugging your ears would be called for.

Have also heard rumors that the .50 will break your shoulder (might bruise you a bit without a muzzle brake, but otherwise, no dice). And that the .50 will detach a retina. (If you're a diabetic, maybe. But if you're a diabetic, there's a list of strenuous activities you should avoid, like demolition derby.)"

Happy to share.

yeah, several times i have to stop squeezing the trigger and put my teeth together as i remember just an instant before regretting it

ok here goes... once upon a time the 50 cal fired from the 0h-58d kiowa would occasionally blow out the chin bubble glass in the cockpit from the muzzle blast. Solution, shorten barrel so it it even with the copilot door... ta da no problem for the airframe...(thanks engineers) Now the pilots are required by the army to wear double hearing protection when you fire the 50 with the doors off (ie hgu-56 helmet and foamys/communication ear plugs and the doors are almost always off) After countless rounds going off about 2-3 feet from my head, mouth open or not I have no measurable loss in hearing (we get tested annually and by nature of me typing this my head didnt explode). Sooo BLUF: I would definitely suggest some decent hearing pro but a fifty cal isnt a howitzer....

P.S.- Make sure you safe the aircraft before you hang out the door to see if there are still rounds in the chute (the counter isnt all that accurate and even though it wont kill you will sure as hell wake you up)

I'd say it depends. One range I used to shoot at had benches on concrete and an insulated, gabled roof of sheet metal. With the new roof, anything over 9mm generally caused me some discomfort in my ears and my teeth felt it with rifles. .223 had me letting my jaw hang. Now, out over dirt/grass, I have no problems. Even the concrete wasn't bad before they put the sheet metal on the underside of the insulation. The underside used to be chicken wire holding up the insulation with metal sheeting on top. I don't shoot there anymore partly because of this... Can't imagine a .50, but I was witness to a .475 nitro express being fired there. I'm not sure whether concrete and the roof really mattered. I could feel the concussion in my feet through the concrete.

Of course you should have your mouth open when shooting .50BMG with no ear protection. Makes it easier for the blood to drain out when your eardrums explode.
