Stoeger coach choke problem

Me and my buddy went hunting today and he took his stoeger coach supreme 20 guage. After shooting a few rounds it looked like something was in the right barrel. Upon further inspection and removing the choke tube, the choke got turned almost inside out. It looked like a shot had hit the front of it and it curled it back all the way to the threads.

He asked me to post here and see if this was a barrel problem or a choke problem. Also if this was common what causes it.

I will see if I can get some in a day or two, we are going hunting again soon so I will see him.
 
I'm thinking maybe there was some kind of ding or deformity in the edge of the choke tube that stuck up enough to catch some shot.
 
He is going to contact the gun store to see if they will replace the choke ($25) and see if it happens again.
 
If the choke wasn't totally put in tightly, then there is a good chance that the blow-by would do that. Luckily, no one was hurt - it could have split the barrels and caused serious injury
