You are mixing two different "forms" and purposes.. The Security Clearance request is not a job resume. Currently the Government is using a resume-type approach for hiring. Investigating one's background for a clearance is a different matter (and form).
On government forms, especially those involving a security clearance, omitting any significant affiliations (Sam's Club doesn't count) is a no-no.
