1. I do now, always. When I first started carrying, no.
2. I'd say 80% of the time I carry my Colt Commander. When I need to REALLY conceal I'll carry my Glock 19 at appendix carry, where it disappears.
3. C&L for the Colt, N/A for the Glock.

An empty gun takes longer to get into action. In a life-threatening situation every split second counts. Why add another disadvantage?
What if you rack the slide and the first round misfeeds? What if your weak hand/arm is injured before you draw your weapon and you CAN'T rack the slide? Too many things can go wrong and burn valuable time.

Not only do I keep the chamber empty, I keep the magazine out of the pistol, that way I can't forget that it needs to be cycled before being ready.
No, I am not 100% happy with this system I am using. I would prefer a DA/SA pistol with decocker and manual safety with a full clip and one in the chamber for pocket carry. But I carry the kahr because it is the smallest lightest 9mm that I can find(not counting a rohrbaugh). Try finding me a DA/SA decocker in 9mm that is smaller than a kahr PM9.
I am seriously thinking about switching to a derringer or maybe a keltec 32 auto. I am not likely to put up with carrying anything bigger than a kahr PM9, and even that gets left in my vehicle half the time.

Every gun I carry will fire when I pull the trigger. Those that have safeties are only used as decockers, safety always off. Chambered round of course.
This is why I stopped carrying my 1911 and Hi Power, so as to not mess up systems under stress.

1)I probably won't have two hands available, at least one will be busy getting my wife and/or son out of the line of attack.
2) It's a 1911, but I carried an XD for a short while and had no real objection to not having a thumb safety, as the pistol was designed that way.
3) It's there, so I use it. One more check-in to make sure the loud noise and travelling projectile are what I really want. I don't consider it a necessity on a carry gun, though, as a holster should cover the trigger and you finger shouldn't be there unless you mean it.
