hi this is my first sbr could you help me with a few questions.

1. for type of firearm do i put sbr or ar15

2. i know i need duplicates of everything my forms came in 3 sheet sections which if i feel out 2 of these these ill end up with 4 forms. common sense says ill only need 1 of these or 2 single sheets is that right?

3. heres what i got am i missing anything?
application to make and register a firearm.
blue/white finger print card.
certification of compliance with 18 u.s.c. 922(g)
200 po money order (is that ok or is a check prefered?)

i did research but conformation is appreciated. thanks ahead of time

If you are asking about box 4 b then that will be "Short Barreled Rifle"

You only need to send in 2 form 1's. Its good you have a extra one in case of a mistake or for your next build. Or you could fill out 3 and send in the best 2 if you have bad hand writing and keep the 3rd for your record.

I had a dealer that would give me 4, 2 were for the ATF one for my records and one for his records in case anything was lost in the mail.

Postal money order is fine, the only bad thing is I don't know if you can go on line to see when it is cashed. Many people go to nfatracker.com and track where the pack is being approved.
