I use one to conceal a S&W 640 or Taurus PT145.
Pro's
Conceals really well.
Additional space to carry reloads
Space to carry ID, money, etc..

Draw is slower than a IWB/OWB holster but is still pretty quick.
Reholstering is almost impossible without opening your pants.
Can be ackward to go to the bathroom (I usually use a stall - otherwise it looks like I'm playing with myself
)
Velcro backing can cause some discomfort on your back (occurs if you don't line up the straps correctly)
Comfort level (next to family jewels)
Not for someone with a beer belly
Sitting can be ackward

They're good for carrying when you're wearing something that can't hold up a gun belt - like gym shorts and a tshirt. Or for when you need to be discrete - like working a job where carry is "against the rules" but 100% legal, and a belt holster would be too likely to be made.
They're good for an "everywhere" gun, because it's pretty comfortable, and you can keep it as a backup when you've got your shoulder holster on.

It's not uncomfortable, I carry it with a Taurus 85 and sitting down is no problem. It does NOT point at anything but the floor. It's like wearing a fanny pack, inside your pants. Imagine an apron with a pocket over your crotch, now put your gun in your pocket, pointed down. Same thing, but inside your pants.
I was skeptical. But SmartCarry offers a money back guarantee, and it's only $50, so I tried it. Within a week, I had figured out how to adjust it comfortably, and it works great.

I like mine. If you're wearing it correctly, it's not going to point at your soldier, the weapon will hang in front of it and point at the floor. I was a little nervous at first about sticking a gun on the captain, but I wear suits often and there really isn't much other option.
I keep a Glock 27 in mine, but mind you, I do not keep it chambered. I'll need the extra 1/4 of a second to rack the slide. It's a fairly comfortable carry method but the fact that it must be drawn or reholstered under the pants crowds the operation so it makes me leary of a Rule 3 violation. Keeping the chamber empty makes me feel better.

The only problem I have with mine is that if the pants are fitted too tight the barrel will push into the bottom of the pants crotch when I sit and the butt will push into my stomach. Spread your legs out when you sit and look at the "V" of the pants - if you have some room there it should be fine, but if it's crowding up on your manhood you should wear it with other pants (unless you won't be sitting). Suit pants are great, tight jeans are not so great.

Wear one on the side like a regular holster. The belt holds it up without weighing down your pants. You can wear it as high as you want or if deep cover is needed, stuff it below your waist line. Tuck over it if you need or leave it out and wear loose clothing.
It's not in the way when you use the'john' when you keep it on your side. You also get a mag pouch built in.
I use mine regularly, with my SW1911. I highly recommend it, especially when turned to the side.

OK, so I am walking one night, some thug appears, approaches, then pulls a knife and demands my wallet. I respond by sticking my hand down my pants toward my crotch. So will he stab me because he thinks I am going for a weapon, or because he thinks I am about to molest him?

For some folks it probably makes good sense, but I'm not going to carry Willie anywhere near MY Willie. I'll stick to strongside pocket carry..Essex
