Concerning the mags on my 75BD, I noticed the two mags I've been using don't have that cutout on the plastic follower for the slide release. I've since changed those followers out, but would that have an effect on extractions?

Maybe. Try changing one mag back and seeing if the problem goes away.
Other than that, extraction issues with the CZ's ar often solved by removing the extractor and cleaning underneath it, replacing the extractor spring with a more powerful spring from Wolf Springs, or replacing the extractor.

Mags hardly ever have anything to do with extraction issues. Id be curious to see how this works....
The cutout on the follower will activate the slide stop/or not..thats all.

I just got through with identical problems nagging me on my BD. Call CZ and get a new extractor with a Wolff spring. I' did so and have been absolutely problem free for 400 rounds since the swap.

There was a problem with extrator springs a long time ago, and a rare case of fouling under the extractor claw might case it to FTE. I have never had this problem with any CZ or CZ clone I owned, however. I hope you get it all squared away - CZs are great pistols!

I put a Wolff extractor spring in my 75BD and my problems went away. No big deal. I always put a Wolff spring in my AR-15's every time I get a new one.

I agree with Terminus. Get a new extractor with a wolf extractor spring and your problem should be solved. That' what I had to do with my 75b and it runs like a top now. Mark

FTEs are sometimes ammo related.
You aren't shooting S&B ammo, are you?
Mine FTEs with S&B to the point that I sort out the S&B brass when I reload, and throw it away.

My CZ75b in .40 S&W totally and utterly HATES Winchester White Box. In my CHL class I got to be the bozo who's gun kept jamming because of this stuff. I've shot it before and didn't like it and vowed never to shoot it again. Of course, on the weekend I'm taking my CHL class, I couldn't find my normal ammo and had to resort to this foul cr*p. (BTW: In case you haven't figured it out, I really don't like that stuff)
Ah, there's one in every class. Sorry it had to be you in your class. That's why I banned Wolf ammo and reloads from my classes. Since then, I've seen no ammo related problems with my students.
I use my CZ 75B as one of my loaner guns for students. So far, no problems,even with WWB.

Funny thing is, I can almost guarantee that if I had been using my reloads, I wouldn't have had a single problem. I've shot about 400 rounds of my reloads through my CZ and not had a single FTE. This with using reloaded Blazer Brass.
Just the luck of the draw, I guess.

I don't like S&B either. With my XD-9 I had a 8% FTE rate with S&B and a 2% FTE rate with WWB so I avoid both of those in all my 9mms now.

I shot .45ACP Wolf in my CHL re-qualifier since I'd lose the brass and knew it was reliable in my gun. No failures. The bozo with the jammer was shooting a Springfield XD next to me. Didn't note the brand of ammo, but it was brass cased.
Not all guns work with all ammo and vice-versa.
My wife and I shoot a lot of Wolf (polymer coated) and Monarch (lacquer coated) 9mm in our CZ 75B with no problems and I only clean it every 1-1.5K rounds.
I was under the impression that TX CHL rules require 32 or larger caliber with factory (from a licensed "manufacturer") ammo in original boxes. I don't believe instructors have the power to ban or require specific brands. OTOH, you are supposed to fail if jams cause you not to get off enough shots in the timed strings to shoot a passing score. I had this problem on my initial qualifier when the extractor on my Colt GM broke (this was before Wolf was on the market so I was shooting UMC if I remember correctly). I had a back-up gun with me but wasn't allowed to switch. Fortunately I'd shot a perfect score up to that point, and managed to get off enough shots single loading and shaking out the empties to pass. I was quite happy with my performance under this "pressure".

(I now have 900 rounds through my XD, using WWB 100 round value packs, with that 1 failure at around round 80)
I have about 300 or so rounds in my Hi-Point using WWB 100 round value packs, failures were due to the nose of the round pointing down instead of up, its a magazine problem not ammo.
I also have 200 rounds through my CZ75 Compact, again using WWB value packs.
