What does FFL paperwork look like?

I'm selling a rifle to someone in California. I received a signed piece of paper but I've never done a person-to-person out-of-state sale before, and I want to make sure I got the right thing. The buyer is an FFL so he filled it out himself and sent it to me. I don't doubt that he's legit, but I want to be 100% sure we're doing this right. Anyone have a link or a scan of what the form should look like? Thanks.
 
I believe your dealer is supposed to have his FFL on the wall of his shop for you to see, but just in case, it should be about 1/2 page long (8.5"x~6"), with the ATF seal about 5" in diameter in the background (on the original it will be blue), the signature is supposed to be in a non-black ink so it cant be photocopied (but some dealers will still take a black signature, while others will ask you send them a new copy signed in blue).
