AIWB doesn't work well to just leave there. Have to unholster IME to get pants off. Some behind the hip holsters also come off because they only stay in place under tension, so will fall to the floor if I just leave them there when taking off pants.
So, many times the holster comes off, nothing to do about it.
Sometimes, there's no stall with enough concealment to leave the gun on the belt as our stupid off-the-floor stalls would leave it visible, and there's no waiting, etc. for a better one.
YES, loaded guns stay in holsters.
Always, always, somewhere I won't forget. A good place is as said, on my pants, between the ankles. Another example, at home where I still don't want to forget it and let kids get to it, is in the sink. I always wash at least a bit, so won't forget there either.
