You can no longer download the application for a C&R license from the ATF website.
As I understand it these old forms are no longer being accepted by the ATF.
On the website you are advised to have the new application form mailed to you and use the form you are mailed.
If you are a Veteran of the Armed Forces, I believe membership in one of veteran organizations such as The American Legion, Veterans of Foreign Wars (VFW), Disabled American Veterans (DAV) would fulfill the "affiliated club" requirement.

just picked up a "new condition" SKS rifle, P prefix, at SAMCO last Thursday - $209.00 (with all new accessories included). I decided to get one with the logbook for $20.00 extra - set another rifle, a "D" prefix rifle (also with logbook) aside for pick up later on this coming week - didn't have...
Nice rifle... bands on mine are loose too (actually, the rear's just hanging there - the rear spring on mine was broken off the pin) ... gotta get that rear band spring... maybe when I order another rifle I'll see about this.
