So right now I carry my full size M&P in a Comp-Tac MTAC I carry it at about 3:30 position. I have breifly played with putting my MTAC in the small of the back position. Long story short I have never carried it there for more than about 10min in the house because the grip of the gun points the wrong way so I would not be able to draw with my right hand.

I have recently come across a left handed MTAC for a very cheap price, is it worth using that as a small of the back holster?

I have always heard horror stories of guys falling on there butts and having the gun break their spine. Is this really a reasonable fear?

Any other disadvantages I should be aware of?

WIth all due respect, use the search. Topic has been covered to death. And as recently as a week or two back. (Twice, I think.)

Summed up:

1) Hurts to sit in a car or seat.
2) Prints HORRIBLY if you sit, bend, or move. (Worst possible position to carry for "printing" concerns.)
3) Your cover garment can very easily ride up over the gun and hang there without you feeling it or noticing. Hey, at least you're not "printing" anymore!
4) Draw stroke can be dangerous if you aren't careful of sweeping your body, legs, feet, etc.
5) Draw stroke is more easily blocked or locked-up becasue you're starting out by getting your strong arm so far behind your body.
6) Falling on your gun could definitely cause serious spinal injury. No one who has had back surgury (and is thus especially aware of the discomfort and dangers) would ever consider it.
7) Some folks do it anyway.

I picked up an ambidextrous IWB holster, and I carry my SigSauer P220 45acp in the small of my back. It isn't uncomfortable for me to sit. For driving, I wouldn't carry it there because you can't get to it that easily. While driving, I leave the holster on the pant/belt, and simply put the gun in the coat pocket I am wearing. When I get out of the car, I put it back in the holster.

Then again, I have about 4 different types of carry depending on the time of year/weather and clothing. IWB small of back; Inside coat/Jacket; boot/ankle; or if I am going to a formal function or in a position where the P220 isn't practical, I use my 32acp which fits in a pocket or jacket pocket without too much weight pulling down and making it noticeable.

there is one "pro" to the SOB carry position...the one it was designed for...as it is a special purpose holster.

it's obvious use is discreet concealment at a dressy affair (executive protion detail) where the user is standing (literally) a post. the holster falls in the natural drape of the tux and actually hides better and accesses quicker than the more traditional shoulder holster
